Entry criteria

Who can enter?

Entry is open to GPs, nurses, and other members of the primary healthcare team, members of PCOs, and other healthcare professionals within the NHS who are involved in implementing national guidance in primary or shared care.

Awards

  • Cardiovascular disease—closing date Saturday 30 June 2007
  • Asthma—closing date End of December 2007

Do you have a suitable project?

  • Primary and shared care initiatives to implement national evidence-based guidance can be entered. Example guidance includes: relevant NICE, SIGN, or academic professional body guidance, NSFs, and indicators in the quality and outcomes framework of the GMS contract
  • Initiatives may focus on the implementation of the full guidance, or an aspect of it, within your practice or locality.

Criteria for entries

Entries should describe, in a maximum of 1000 words:

  • Why you decided to implement this particular national guidance, or part of the guidance, in your practice/PCO/locality
  • The steps taken to implement the guidance/part of the guidance
  • How the initiative has produced an improvement in evidence-based practice—please note that audit data or equivalent evidence should be included to demonstrate improvements. Examples of measured outcomes from previous winning entries include:
    • Attainment of QOF points
    • Time savings
    • Cost savings
    • Improved diagnosis of disease
    • Audits of care
    • Positive changes to prescribing
    • Success rates of interventions
    • Service uptake
    • Reduced hospital admissions/visits
    • Impact on referral rates
    • Improved patient satisfaction
  • How the initiative has resulted in tangible benefits to patients
  • Any steps taken to roll out the project
  • Key learning points that may be transferable to other organisations
  • Conclusions
  • How you plan to develop the project in the future.

Please also include a brief abstract (an additional 100 words).

Announcement of winners

  • Cardiovascular disease Award 2007: The winners will be announced in the November 2007 issue of Guidelines in Practice. The judges’ decision will be final
  • Asthma Award 2007: Winners announcement to be confirmed. The judges’ decision will be final.

The prize

The winning team for each Award will receive £2,000 to help further their initiative.

Contact details

Please enter the full title of the project, and the name and contact details of the clinical lead, listing other team members in the appropriate boxes.

The abstract

The abstract should be no more than 100 words in length. You may find it useful to divide your abstract into the following sections: objectives, approach used, results/outcomes achieved, benefits to patients, lessons learnt, and conclusions.

The project initiative

The write-up of your project should be no more than 1000 words in length. The criteria for entry listed above can be used for reference when preparing your entry.
